DVA OCCUPATIONAL THERAPY

Sarah Smith Occupational Therapy provides community-based occupational therapy assessment and recommendations for eligible veterans and DVA clients across Newcastle, Lake Macquarie, Port Stephens and surrounding areas.

Services may include assessment for aids and equipment, home modifications, functional needs, safety, access and everyday independence at home.

• Bathroom, access and other home modification assessment 
• Mobility, transfers and daily living recommendations 
• Reports and documentation to support DVA processes

To access occupational therapy services through DVA, a D0904 referral  is required from your GP or medical specialist.

This referral allows eligible DVA clients to access occupational therapy assessment and recommendations under DVA arrangements, including assessment for aids, equipment, home modifications and other supports where clinically appropriate.

Your GP can complete the DVA referral form or provide a referral on their usual letterhead. DVA provides further information about referrals and the D0904 referral form on the DVA website.
