HOME MODIFICATIONS ASSESSMENT

What is a home modifications assessment?

A home modifications assessment looks at how the home environment is supporting, limiting or increasing risk during everyday activities.

What does a home modifications assessment involve?

The assessment considers the physical layout of the home, current access, personal care areas, transfers, mobility, safety risks, existing equipment and the person's functional needs, preferences and goals.


Recommendations may include simple changes such as grab rails, through to more complex modifications such as bathroom redesign, ramped access, doorway changes or other structural changes.

Assessment approach

Recommendations are developed by considering the person, the activity, the environment and the support available. The aim is to identify options that improve safety, access and independence while remaining realistic within the home and funding context.


For complex home modifications, assessment will involve collaboration with builders, building construction practitioners, suppliers, and funding bodies.

Common reasons for referral

  • Difficulty accessing the home, bathroom, bedroom or key living areas
  • Falls risk or reduced safety during showering, toileting, transfers or mobility
  • Need for grab rails, ramps, threshold changes or bathroom modifications
  • Changing mobility needs, including wheelchair or walking aid use
  • Support with documentation for funding bodies such as NDIS, DVA or aged care pathways
